Terms & Conditions

Effective Date: [January 8, 2026]

These Terms & Conditions (“Terms”) govern your access to and use of the CarePoint Infusion Pharmacy website. The website is operated by CarePoint Healthcare Solutions LLC, doing business as CarePoint Infusion Pharmacy (“CarePoint,” “we,” “us,” or “our”).

By accessing or using this website, you agree to these Terms. If you do not agree, do not use the website.

1. Website Purpose

This website provides general information about CarePoint Infusion Pharmacy, sterile compounding, home infusion pharmacy support, provider and clinic services, patient resources, quality and compliance practices, licensure, service availability, and pharmacy contact information.

The website is informational and does not replace direct communication with a pharmacist, prescriber, or other healthcare professional.

2. No Medical Advice

Website content is provided for general informational purposes. It is not intended to diagnose a condition, recommend a specific treatment, determine whether a medication is appropriate, replace professional medical judgment, or replace consultation with a licensed healthcare professional.

Questions about diagnosis, treatment decisions, or changes in therapy should be directed to an appropriate licensed healthcare professional.

3. No Emergency Services

This website and its forms are not intended for emergencies or urgent medical needs. Call 911 or seek immediate emergency care if you are experiencing a medical emergency.

4. CarePoint’s Role as a Pharmacy

CarePoint is a pharmacy. CarePoint does not use this website to diagnose patients, independently prescribe medications, establish treatment plans, or replace the relationship between a patient and a licensed prescriber.

Treatment decisions are made by authorized healthcare professionals acting within their scope of practice.

5. Prescription Medications

Prescription medications are dispensed only after CarePoint receives and verifies a valid prescription issued by an authorized prescriber in accordance with applicable law.

Use of this website does not guarantee that a prescription will be issued or accepted, that a medication will be compounded or dispensed, or that a particular service will be available.

All prescriptions remain subject to applicable law, pharmacy review, prescriber verification, patient eligibility, licensure requirements, professional judgment, product availability, and other applicable pharmacy requirements.

6. No Online Prescription Drug Checkout

CarePoint does not offer direct online checkout for prescription medications through this website.

Submitting a general inquiry or provider form does not constitute a prescription, create a right to receive medication, authorize dispensing, or guarantee pharmacy acceptance.

7. Compounded Medications

When clinically appropriate and permitted by law, CarePoint may prepare patient-specific compounded medications pursuant to valid prescriptions.

Compounded drugs are not FDA-approved. FDA does not review compounded drugs for safety, effectiveness, or quality before marketing in the same manner it reviews FDA-approved drugs.

A licensed prescriber is responsible for determining whether a compounded preparation is appropriate for an individual patient. CarePoint compounds and dispenses medications only as permitted by applicable law and pharmacy requirements.

8. Service Availability and Licensure

CarePoint provides pharmacy services only in jurisdictions where it is properly licensed or otherwise legally permitted to provide the applicable service.

Current service availability is described on the Licensure page and may change. Patients and providers should confirm current availability before submitting a prescription or relying on website information.

9. Website Forms and Sensitive Information

General website forms are intended for general inquiries. Unless a form is specifically identified as an approved secure channel, do not submit detailed medical information, prescription details, medication names, laboratory results, Social Security numbers, insurance identification numbers, or other highly sensitive information.

For patient-specific matters, contact the pharmacy directly or use another communication method specifically approved by CarePoint.

10. Accurate Information

You agree not to knowingly provide false, fraudulent, misleading, or incomplete information through the website.

Providers and organizations submitting inquiries are responsible for accurately representing their identity, licensure, practice information, contact information, and authority to act.

11. Electronic Communications

Email, text messaging, and other electronic communications may not always be secure. Do not use an unapproved communication channel for sensitive medical or prescription information.

When you provide contact information and request communication from us, you authorize CarePoint to respond using reasonable communication methods consistent with your request and applicable law.

12. Prohibited Uses

You may not use the website to violate law; submit fraudulent information; impersonate another person; attempt unauthorized access; introduce malware; interfere with website operation; scrape or harvest information through unauthorized automated means; infringe intellectual property rights; or misuse pharmacy, patient, or provider information.

13. Intellectual Property

Unless otherwise stated, website text, graphics, logos, branding, images, design elements, and documents are owned by, licensed to, or used with permission by CarePoint and may not be copied, reproduced, distributed, modified, or commercially exploited without authorization except as permitted by law.

14. Third-Party Links and Services

The website may contain links to third-party websites, portals, maps, carriers, or services. CarePoint does not control third-party content, security, availability, or privacy practices. A link does not necessarily constitute an endorsement.

15. Website Availability and Accuracy

We make reasonable efforts to maintain accurate information, but services, licensing, policies, and website content may change. Website access may also be interrupted. We may modify, suspend, or discontinue website features at any time.

16. Disclaimer of Warranties

To the extent permitted by law, the website and its content are provided on an “as available” basis. CarePoint does not guarantee uninterrupted access, error-free content, or that every service described will be available to every person.

Nothing in these Terms limits rights or obligations that cannot lawfully be limited.

17. Limitation of Liability

To the extent permitted by applicable law, CarePoint will not be liable for indirect, incidental, special, consequential, or punitive damages arising solely from use of, or inability to use, this informational website.

This limitation does not apply where liability cannot legally be excluded or limited.

18. Privacy

Use of this website is also subject to CarePoint’s Privacy Policy. CarePoint’s handling of PHI is further described in the Notice of Privacy Practices.

19. Changes to These Terms

We may revise these Terms from time to time. Updated Terms will be posted on the website with a revised effective or last-updated date. Continued use after updated Terms are posted constitutes acceptance to the extent permitted by law.

20. Governing Law

These Terms are governed by applicable federal law and the laws of the State of Texas, without regard to conflict-of-law principles. Any legal proceeding relating solely to use of this website will be subject to jurisdiction and venue required or permitted by applicable law.
